Rapid rehousing program for homeless veterans and their families, services include housing search, assistance in negotiating the terms of a lease, short-term rent subsidies, move-in assistance, individualized case management after the individual/family has moved into the home, ongoing crisis prevention/crisis management services as needed, access to community resources and other supportive services.`


Mon-Fri - 8:00am to 4:30pm

Required Documents

- DD214 (an SF180 can be filled out at assessment if veteran does not have proof) - Proof of income - Driver's license or state identification card - Social security card for all household members - Birth certificate for children in household


- Military service of at least one day beyond initial training - Homeless verification will be conducted by St. Patrick Center
