Description

Family Foundations (ESG Homelessness Prevention Program) is a Federal funded program to assist imminent homeless and at-risk of homeless individuals and families achieve and maintain housing stability.

ID, Social Security card, verification of income - Extensive intake and assessment focused on housing stability

Eligibility

Must meet HUD definition of imminent or at-risk of homelessness; must be at 30% lower AMI; must have no other resources or support

Description

Offering financial assistance toward rent and utilities if other agencies cannot be utilized, assistance with car repairs and deductible payments for damaged automobiles. Other assistance will be determined on an individual basis.

Description

Families Forward is a rapid re-housing program for families experiencing homelessness. At this time, program limited to just a few families each year.

ID, Social Security cards for all family members, income verification - Intake application and personal interview

Eligibility

Must be experiencing homelessness; have at least one child under 18 and must have some basic form of income

Fees

No fee to start; rent assistance offered on graduated scale over one year
