THE HOUSING ASSESSMENT IS NOT A GUARANTEE OF SERVICE AND THE WAIT FOR A PROGRAM OPENING MAY BE LONG. PLEASE PROVIDE REFERRALS FOR IMMEDIATE SERVICES LIKE SHELTER, FOOD, ETC IN ADDITION TO REFERRING FOR AN ASSESSMENT.

Description

GKCCEH conducts a housing assessment for people who are homeless to determine priority for permanent housing and housing payment programs. This assessment is used to add people to a prioritized list for supportive housing programs which offers openings to the most vulnerable people first. Vulnerabilities assessed include how long someone has been homeless, their health conditions, domestic violence history, and other factors.

GKCCEH office: M-F 9am-5pm; pre-screening completed 24 hours through 211

None for pre-screening and GKCCEH assessment, other services/programs referred to may have documentation requirements; must be able to provide a contact phone or email for GKCCEH to follow up

Must be literally homeless or fleeing domestic violence

Intake Procedure

Complete Pre-Screening by calling 211 and speaking with a call specialist. Call specialists will enter pre-screening answers in database which, if eligible and with caller permission, will be sent to GKCCEH; then GKCCEH will follow up with the caller to schedule an assessment in approximately 4 business days.
