                                                    Description

                                                    DCFS High Risk Wards: The Pathways program provides information and support, includes health referrals, nutrition education, prenatal self-care information, including labor and delivery, child development information and education (prenatal to age seven), parenting skills enhancement, developmental screenings, video Baby Book, childbirth education, independent living skills, intensive case management services for the teen and her family, advocacy, crisis intervention educational support and monitoring college/vocational job search assistance, counseling and recreational/cultural outings. All services are conducted in the home, home visits are conducted weekly or more frequently, based on need and a Participant Support Plan is developed with the teen.
