Description
Care Management and Transitional Care: Specialists provides coordination of follow-up medical care, outpatient therapy appointments, durable medical equipment and medical supply needs, medication management, transportation issues from one facility to another, help with insurance and payment issues, grocery, banking, bills, laundry, etc., advocacy support in difficult hospital situations and develops a personal care individualized plan of action.

Adult Care Monitoring and Ongoing Support: Professional registered nurses and geriatric care managers evaluate the care received by an elderly or disabled person and provide ongoing interventions to improve the care, monitors in-home helpers, nursing home and hospital care reviews and timely adaptations as situations change, coordinates multiple helping agencies, staff members, formal and informal arrangements.
