Intake Procedure
Must speak with local Lion's Club and they Lion's Club will fill out the paperwork in order to apply for assistance from Mid-South Sight. Cannot apply without the assistance of your local Lion's Club.

Through partnership with area Lions Clubs, AFL-CIO Community Services does intake for eyeglasses for adults.

Proof of income, proof of address
Eligibility
Income guidelines, must be uninsured and can only get eye glasses every three years.

Intake Procedure
Call, email, or walk in. Once the application is complete, AFL-CIO Community Services will provide a referral for an eye exam, and then forward paperwork to glasses provider. Use SNAP income guidelines.
