      Description

      Foster Care Case Management: Foster care case managers provide, coordinate and facilitate all services necessary to support the permanency goal for the children in care. Case managers provide for safety and needs assessments, resource acquisition transportation and visitation, court reports, and authorization of funds for services to family members.

        Description

        - Residential Treatment Program: Offers individual, group and family therapy, psychiatric services, medical services and 24 hour supervision based on each child's individuals needs. - Residential Treatment Services Self-Injury: Offers much needed calm from the storm of self-injury, with numerous therapeutic activities and groups, recreation, and opportunities to practice learned skills in a safe supervised environment. - Community Counseling Services: Community-based therapeutic- programming provided.

          Description

          Changing Prism: A year-round intervention that focuses on the needs of young women overcoming trauma. There are 12 regional events that take place throughout the year and focus on building self-confidence and goal setting, as well as mental and physical health.

            Special Notes/Funding

            This program is helpful for children struggling with regulating emotions or disruptive behaviors, who need more hands-on intervention than can be provided in outpatient therapy.

            Description

            Intensive Outpatient Program (IOP): Treatment is based on each child’s individual needs and a strategic combination of these therapeutic services are applied individual/group/family therapy, psychiatric services, medical services and direct supervision.

              Description

              - Intensive Family Intervention Services: Provides in-home services to children for 45 days to help them successfully reintegrate into their families, homes and schools after a period of 24-hour inpatient care. Services include intensive in-home parent support and education, care and service coordination, 24 hour crisis intervention and intensive home support. - Intensive In-Home Services: A short-term, home-based, crisis intervention service offering families in crisis an alternative for remaining together safely, and averting out-of-home placement of children whenever possible. The program combines skill-based intervention with maximum flexibility so that services are available to families according to their unique needs.

                  Families or individuals who would like to become a foster parent must meet the following criteria to qualify: - Must be at least 21 years of age - U.S. citizen by birth or naturalization, or be able to provide lawful immigration status - No criminal or child abuse/neglect record - Financially stable - Good physical, mental and emotional health - Home must be a safe environment for children - Available space to accommodate children - Successfully complete all training requirements (if married or co-habiting, both must attend all required trainings) - Participate in the home assessment process to become a licensed foster home - Foster parents can be single, cohabitating or married - Foster parents can own their own homes, or rent an apartment

                    Special Notes/Funding

                    The purpose of Great Circle’s Aftercare program is to provide rehabilitative services to children in residential treatment that help them move from the treatment facility to a home or community-based setting.

                    Description

                    Aftercare Program: Provides a family assessment, monitoring and ongoing management of a medication regimen, therapeutic and clinical treatment services, as well as hard and soft support services for the family, teachers and community members to support success in each area.

                    Availability

                    Program was designed as a 120-day program with 30 days in a residential facility and with 90 days of services provided in the home after the child’s return

                      Description

                      - Partial Hospitalization Program (PHP): Allows children to maintain current placement while still engaging in intensive treatment. Treatment is based on each child’s individual needs and utilizes a combination of therapeutic services, including individual, group and family therapy, psychiatric services, medical services and active, direct supervision. - Day Services: Program coordinators provide initial orientation and commitment to program components, identify and address barriers to comprehensive treatment, and collaborate with existing community treatment providers and school personnel. Ten hours of group therapy each week focuses on dialectical behavior therapy skills building, strengthening and generalization via empirically supported therapeutic learning activities.

                        Description

                        Children and teens are placed in private, licensed homes that provide therapeutic, highly structured care within a family setting. The client then lives in the Therapeutic Foster Care family home and attends school within the family’s community. Case managers conduct weekly face-to-face meetings with the child or youth and provider.
