Description
Vibrant Health provides preventive and acute medical services and chronic disease management for patients of all ages; lab services; care coordination and specialist referrals; prescription assistance; and Medicaid enrollment assistance. Transportation to and from clinic visits may be available for established patients.

MTh 8am-5:30pm, TF 8am-5pm, W 7:30am-5:30pm

A photo ID, proof of address and proof of income for all household members.

Sliding fee scale for uninsured patients. Medicaid/Kan-Care, Medicare and most private insurance are also accepted.
Intake Procedure
Call for appointment; no walk-ins. Expect a 1 week wait for new appointment.
Languages
Interpreters are provided for patients & patient families who are proficient in a language other than English.

Integrated referrals to the Vibrant Health- Central Dental Clinic are made for hygiene and restorative services. When needed, Vibrant Health dental staff coordinate additional external referrals for specialty dental care.
