2-1-1 Housing Plus - After Hours

Description

2-1-1 connects adults to shelter seeking shelter 24 hours a day, 7 days a week. 2-1-1 completes Homeless Service Prescreen assessment to determine eligibility. If a person is calling for someone else, as long as the person who needs shelter is nearby to answer questions then we can assist. Offer general information about after hours - if the person is determined eligible then we will connect them to an on call after hours person for shelter. City - If a St. Louis City Police Officer or a Hospital calls for someone, go ahead follow steps 2 and 3. County - If St. Louis County Police Officer or a Hospital calls for someone, go ahead and follow steps 2 and 3 (even if it's not after 7pm)

St. Louis City - After Housing Plus closes and on weekends St. Louis County - After 7pm Mon-Fri (between 5pm and 6:59pm, tell people to call back after 7pm) and Sat - Sun after 7pm (before 7pm Sat and Sun, tell people call back after 7pm)

Eligibility

1. Identify being homeless, needs a place to go, is being kicked out, needs shelter, or otherwise is looking for homeless shelter resources. 2. Complete the Homeless Services Prescreen questions in the primary assessment. 3. If the person DOES NOT have a safe place to stay, a. Person is in St. Louis City - Check Get Help for available shelter beds. If there are no available shelter beds, go into the inContact address book and use the St. Louis City After Hours Emergency Line. b. Person is in St. Louis County (MUST HAVE LAST RESIDED IN ST. LOUIS COUNTY VERIFY THEIR LAST ZIP CODE WAS IN THE COUNTY) - place the person on hold and call 314-279-7036, identify that you are with 2-1-1 and have a person who does not have a SAFE PLACE and needs shelter TONIGHT. Let them know how many people need shelter and warm transfer the 2-1-1 caller. If there is no answer and you are able to leave a message, do so. Voicemail: If there is no answer and you able to leave a message, do so. Include your name, that you are with 2-1-1 that you have a person who/family who does not have a safe place to stay tonight. Give them the number of people, the client's name and Date of Birth, and the client's callback information.
