Required Documents
If requested, must provide letter of diagnosis from physician, must be able to provide current tax return to prove dependent status, and must be able to provide additional data and documentation to support various information in the application.
Eligibility
Must be undergoing active treatment with an established oncologist within the last 12 months. Must have dependent children under the age of 18 living in the home

Intake Procedure
Must complete the application in its entirety. The Director of Financial Aid will screen each application and schedule a personal (in-home) interview. Applications and interviews will be reviewed by the Board of Directors at a monthly board meeting. At that time, applications will either be approved or denied. The Director of Financial Aid will contact the applicant to deliver the board's decision.
