Photo ID, or two alternate forms of identification may be accepted if an applicant does not have photo identification: Must include the applicant's name and the name of the organization the document came from, such as a pay stub or utility bill.
Eligibility
Vital records certificates are issued only to the person named on the certificate or that person's parents, must be named on certificate, siblings, grandparents, aunts, uncles, children, or legal guardian. The local registrar is responsible for determining requests on a case-by-case basis.

Sliding scale; Fee based; VFC is sponsored by the CDC to offer a limited supply of vaccinations (including influenza) for those that have Medicaid, uninsured, and underinsured children age 6 months to 18 years. LCHD also accepts private insurance. Insurances accepted Aetna, Blue Cross Blue Shield, Cigna, ChampVa, Coventry, FMH Benefit Services (Coresource), Humana, Medicare Part B, Meritain, Tri Care, UMR, and United Health Care. Please bring your card with you and we will check to make sure immunizations are covered.

Description
The Lafayette County Health Department is now a certified car seat safety inspection station. Our certified technicians are certified to check all kinds of car seats to make sure each child is in the correct kind of car seat that ensures the best fit for the child. The CPS technician's purpose is to educate parents on how to install a car seat properly and to use the proper car seat based on each child's specifications.

Description
Classes are held at the Health Department, 547 S 13 Hwy in Lexington. The basic class is approximately 1 1/2 hours. Classes may also be scheduled on-location for new businesses and in special circumstances. Classes may be scheduled on site if 10 or more students can register and pre-pay. All employees and management with food preparation duties are required to obtain a Lafayette County food handler's card within 1 month of hiring.

Please call 1-660-259-4371 x2014 at least 24 hours in advance to pre-register Provide name, drivers license #, and workplace for each applicant to pre-register. If you are registering a group of people fax a list with the required information to 660-259-6250. Failure to properly pre-register will result in a delay in the issuance of the food handler's card.
