Completes a standardized spiritual wellness assessment. Based on the results of the spiritual wellness assessment, an older adult can receive 6-12 sessions of spiritual counseling over the phone from a Chaplain. Spiritual counseling includes the following: Praying with the participant, reading Bible passages, assisting with grief/loss of a loved one, and connecting them with a spiritual community.
