Vital records certificates are issued only to the person named on the certificate or that person's parents, must be named on certificate, siblings, grandparents, aunts, uncles, children, or legal guardian. The local registrar is responsible for determining requests on a case-by-case basis.

$21 initial vaccine fee and $15 for each additional vaccine for uninsured or underinsured, no sliding scale, payment plans may be available, Medicaid and most major insurances accepted.

Services include nutrition and health education, a reloadable card to buy healthy foods for parents and their children, personal counseling about nutrition, support and help with breastfeeding and referrals to other health programs.
