Description
Case plans are made for homeless individuals to assess needs, coordinate needed services, ensure services attained and to follow up and monitor progress as many have severe and persistent mental illness.

Eligibility
Diagnosis of severe and persistent mental illness (SPMI) or suspicion of SPMI and must be homeless with definition of homeless individuals living on the street or in shelters (staying at someone's house, not their definition of homeless)
Fees
Medicaid, Medicare and most types of commercial insurance. Uninsured residents of Jackson County and Kansas City, Missouri can receive services on a sliding fee schedule based on their income/resources.
