Description

St. John's Adult Day Program: Enjoyable social setting for individuals challenged by memory loss, dementia, and/or physical disabilities, lunch and nursing oversight included as well as variety of activities, exercise and entertainment. Also, provides respite for family caregivers.

Hours

Mon-Fri - 6:30am to 6:00pm

Required Documents

- Upon enrollment will need to share medical insurance cards (for use in medical emergency) - Power of Attorney forms and/or Do Not Resuscitate order if applicable

Eligibility

- Adults age 18 years and older who are challenged by physical or mental disabilities - Must be able to help transfer self to chair and must not be a danger to others - Anyone in need unless they require lifting, constant one-on-one care, or are a danger to staff or other participants

Fees

- $13.50/hour, four-hour minimum - Some partial scholarship assistance available - call for details

- Forms available by mail, on website, or in office - Individuals eligible for Illinois Community Care Program or Illinois Division of Rehabilitation Services must apply through case manager, St. John's staff available to help as needed
