M-F 8:30am-4:30pm by appointment
Required Documents
Proof of income and address, ID & social security card for household members
Eligibility
Requires source of income, will serve undocumented persons
Fees
With discount, the first appointment is typically $76.40, takes active Medicaid for children; Sliding fee scale for adults (based on income and number of dependents). Qualified individuals may receive up to a 60% discount for exams and x-rays.

Call for appointment. No walk ins please.
