Locks of Love accepts hair donations and makes hairpieces for financially disadvantaged children under age 21 suffering from long-term medical hair loss from any diagnosis.

Required Documents
Hair donation form or simply include your name and email or mailing address.
Eligibility
Hair donation must be 10" or longer, clean and dry and bundled in a ponytail or braid. No bleached hair.

Application, doctor's diagnosis (may be faxed from doctor's office), photo of child without hairpiece or hat, proof of income, two personal letters of recommendation explaining why the child would benefit from a hairpiece.

Youth aged 21 and younger who are financially disadvantaged and are suffering from a long-term medical hair loss from any diagnosis.
