Required Documents
New members are encouraged to bring a doctor's note indicating the recommended amount of weight to lose
Eligibility
Persons age 7 and older who want to lose weight.. Females served, ages 7 to 150. Males served, ages 7 to 150.
Fees
Membership dues are $28 per year
Intake Procedure
Self-referral by phone, mail, Internet; can also come in to meetings
