Intake Process:
Complete intake paperwork by visiting our office at 701 S. Joplin Avenue or downloading from our website at http://joplinclinic.org/services.htm. Must provide a photo ID, proof of income (our state assistance), and proof of residence (piece of mail, etc.).
Program Fees:
Medical services have no fees though we do accept donations from patients. There is a $20 fee charged for dental x-rays and extractions.
Eligibility:
Must be uninsured for medical services. For dental extractions, must not have access to dental assistance.
