Description
Provides services to individuals with severe disabilities so they can remain in their homes, services may include in-home personal assistant, homemaker services, maintenance home health, electronic home response, home delivered meals, adult day care, assistive equipment, environmental modification and respite services.

- Persons with severe disabilities remain in their homes - Persons under age 60 years at risk of moving into a nursing home or other facility
