Description
HospiceCare, Inc. provides palliative medical, nursing, social, spiritual and bereavement services to people with a terminal illness and to their loved ones.

Required Documents
Must have an order from a physician before a person can start receiving hospice services.
Eligibility
Certification by a physician that the patient has a terminal illness with a prognosis of six (6) months or less if the illness runs its normal course
