Provides end of life care to individuals that are terminally ill by using a team approach of nurses, aides, physicians, social workers, chaplain and volunteers.

- Client's physician must certify that they meet hospice criteria of having terminal illness with six months left to live - Medical/Psychiatric records
Eligibility
Individuals with a life expectancy of 6 months or less and a decision to focus on comfort care (rather than curative) with pain control and symptom relief.
