Description
UBCF offers and Individual Grant to breast cancer patients, which can be used for medical expenses such as procedures, prescriptions, nutritious food or supplements, and COBRA insurance payments. All funds are disbursed directly to provider.

Application; Proof of income; Doctor's Letter of medical condition; Any outstanding bills, household bills, or estimates for cost of treatment
Eligibility
Income is taken into consideration (see application); Must be current breast cancer patient or within 3 years of remission
Fees
$25 Pay It Forward Contribution due by check at time of application.
Intake Procedure
Call or visit website for application. Applications are reviewed on a quarterly basis. You will be notified by email. Every applicant receives assistance.
