New Eyes for the Needy provides eyeglass vouchers for children and adults to purchase glasses online.

Required Documents
Prescription (including pupillary distance), proof of income or assistance
Eligibility
Income guidelines (200% Federal Poverty Level or less); must have had an eye exam including pupillary distance; must have no other resources available to them to pay for glasses

Intake Procedure
APPLICATION MUST BE COMPLETED BY CASEWORKER. Application is available on the website. The voucher will be emailed directly to the caseworker and can be used at New Eyes for the Needy's online glasses dispenser at www.neweyesglasses.org.
