Eligibility

Under the care of a physician who signs and orders the home health plan of care; homebound; requires an intermittent skilled service - nursing, physical, speech or continuing occupational therapy.

Fees

Medicare, Medicaid, or private insurance. If home health care is not a covered service arrangements can be made for a self-pay option if the patient qualifies.

Intake Procedure

Call for intake. Doctor can fax or phone in referrals. A patient referral must include physician orders for home health care.
