Required Documents

- Applicant must present a Picture ID - Must provide Social Security cards for every household member - Persons under age 60 years must provide a letter from their doctor containing a specific ailment a household member is suffering from or have children in the household five years and younger regardless of an existing medical condition - Must provide a current copy of utility bill to prove that they have utilities and do not have a back bill and are not in threat of disconnection (utility bill must be less than $500.00) - Income restrictions exist, call for details - Must provide income documentation for their entire household/family income for the past 90 days - Everyone over the age 18 years must provide income documentation

Eligibility

- Must be age 60 years and older or suffer from a medical condition that can be relieved by air conditioning - If any household member received an air conditioning unit in the past three years, they are not eligible to receive an air conditioning unit in 2014 - Applicants will be required to show a picture ID when they pick up their AC unit, only the person who made the application will be allowed to pick up their unit

- Required documentation must include a doctor's certification letter, dated within ninety days, stating that the medical condition would be aggravated by extreme heat for households that contain a member with a medical condition - Required documentation must include a letter of determination from the Social Security Administration or receipt of Social Security Disability benefit statement for households that contain a member receiving Social Security Disability benefits or with a pending determination case at the Social Security Administration

Eligibility

- Low-income at-risk households - Households that contain a member with a medical condition that can be improved by air conditioning, such as asthma, emphysema, coronary disease and terminal illnesses - Households that contain a member that is elderly age 60 years and older - Households that contain children less than age 60 months as of the date of application - Households that contain a member receiving Social Security Disability benefits or with a pending determination case at the Social Security Administration - Current PIPP (Percentage of Income Payment Plan) participating households that are receiving PIPP benefits on their electric ARE NOT eligible for summer cooling assistance - Also households that received PIPP benefits anytime during the period of September 2013 through August 31, 2014 ARE NOT eligible for summer cooling assistance - Income guidelines exist, call for details
