UHON Statewide: Provide financial assistance with one month of child care payments for state-licensed childcare provider.

Required Documents
Photo ID Proof of Residence (at least 1 utility bill, personal property tax statement, tax return, bank statement, letter from government or court, statement of government benefits like SSI/SSDI, WIC, or SNAP) Proof of earned and unearned income for all household adults (at least 1 for each adult - 2 recent pay stubs, letter from employer, pension letter, Armed Forces Allotment, Child Support, Union Funds or Strike Benefits, Government benefit statement letters like SSI/SSDI, TANF, Unemployment or WIC/SNAP, tax returns) Proof of Social Security Number for all household members (SS cards, award letters, tax return with SS Numbers for all household members)
